Hospital To Home (H2H)

Covered Caregiver provides a trained, insured and healthcare-credentialed caregiver to assist a discharged patient settle right back into his or her home.

How Does It Work?

STEP 1 – Whether you or your family have reached us by phone or email; or referred to us by a case manager or doctor, a Geriatric Care Manager along with one of our esteemed caregivers schedules a visit to your home.

STEP 2 – The Covered Caregiver team performs an interview and care assessment with the goal of designing a custom care plan. This is a time where all your questions will be answered, concerns put to ease and a chance to build rapport with that particular care provider.

STEP 3 – Your custom care plan will determine what services are required to meet you or your loved ones need. Services are offered and performed from as little as 4 hours per day and up to 24 hours, 7 days a week for more challenging conditions, such as dementia.
